To register your child, you must bring all needed forms and required documents to Central Avenue Elementary School in person.
I. Proof of Residency in School Zone:
A resident parent or guardian admitting a pupil to an Osceola County School shall produce at least two (2) forms of residential documentation.
II. Proof of Physical Examination:
A physical examination must have been completed within the last twelve (12) months prior to day of enrollment. Students having attended a Florida school are exempt when providing documented evidence of a physical from that Florida school.
III. Immunizations:
All student immunizations must be up to date and on Florida Form 680.
IV. Proof of Date of Birth:
State law requires that a child must be five years old on or before September 1 of the ensuing school year to be eligible to enroll in kindergarten. A child must be six years old on or before September 1 of
the ensuing school year to be eligible to enroll in first grade.
